Organization: Medical Psychiatry Alliance

This Foundational course will provide participants with practice and evidence-based approaches to identifying, engaging and managing co-existing physical and mental health needs. Participants will develop an integrated approach to care for these patient issues and will obtain access to practice-ready toolkits to implement in their setting.

Learners who actively engage and complete this course successfully will be able to:

  • Identify the prevalence and impact of mental health conditions on physical health outcomes
  • Describe and use evidence-based assessment tools to improve identification of mental health needs in patients with physical health issues
  • Describe and perform an integrated case formation to address co-occurring physical and mental health needs

Organization: Medical Psychiatry Alliance

The Medical Psychiatry Collaborative Care (MP3C) certificate is aimed at creating integrated care capacity in clinical teams working within both acute and community primary care settings. This program was developed as part of a larger medical psychiatry alliance across healthcare organizations and the University of Toronto. The MP3C program integrates evidence-based models of medical psychiatry (mental and physical health) integrated care, with clinical skills development and quality improvement to shape the delivery of integrated care across the patient continuum, from pediatrics to geriatric and palliative care.

This workshop features an inter-professional approach to managing complexities in patients with response behaviours of dementia, including the topics of differentiating between delirium and dementia approaches to behaviour assessment, caregiving strategies to minimize responsive behaviours and communication approaches. Workshop participants will have the opportunity to reflect on patient encounters and discuss improved patient outcomes and experiences.
